We have a high number of elderly patients and have two registered nurses employed to look after their needs.  We perform health assessments for the population over 75. This is a chat in the patient's home to find out what services and aids might be available to the patient to enable them to stay in their own home longer and to make life easier.  For the patients who are in nursing homes in our area we offer home visits if, and when necessary.

For our diabetic patients we have nurses who are able to discuss their care and needs and arrange referrals to a dietician, podiatrist and ophthalmologist in order to best maintain their diabetic control.  We have a diabetic educator who does clinics at both our surgeries at regular intervals throughout the year. Ask when the next clinic will be held.

Patients with coronary heart disease also have special needs and we have nurses available to discuss their care also.
